Usrgical Treatment on Graves' Disease



Abstract
A clinical study of hyperthyroidism was taken for 73 patients who were operated in the department of Surgery, at Kyung Hee University Hospital from March 1986 to February 1992.
@ES The results were as follows;
@EN 1) Male to female ratio was 1 : 3.3 with an average age of 28.9 years, with the highest incidence in the third and fourth decade(71.2%).
2) Lengths of time from diagnosis to operation were variable but almost of them were within 3 years.
3) Common symptoms were as follows; in decreasing frequency neck mass, palpitation, sweating, fatigue, weakness, exophthalmos, weight loss, tremor, heat intolerance, etc.
4) Preoperative thyroid function test showed elevated serum level of T3 and T4 with a mean value of 405.7ng/dl and 17.4 §¶/dl respectively.
5) In most patients, propylthiouracil, ¥â-blocker and Lugol solution were used preoperatively (64.4%).
6) The types of operation were bilateral subtotal thyroidectomy in 71 cases(97.3%), and unilateral total lobectomy with contralateral subtotal lobectomy in 2 cases(2.7%).
7) In pathologic classification, all cases were diffuse hyperplasia and one of them was combined with Hashimoto's thyroiditis and other one or them was combined with follicular adenoma.
8) Postoperative complications were transient hypocalcemia in 9 cases(12.3%), wound hematoma in 3 cases(4.1%), transient hoarseness in 3 cases94.1%), and wound seroma in 2 cases(2.7%).
9) Operation result were as follows; the decrease of TBII in postoperative period was observed of 14 patients(100%) over 6 months follow up and permanent hypothyroidism and recurrence were developed in 1 cases93.1%) and 3 cases(9.4%)
respectively.
10) Although the preoperative preparation is much improved, more skillful technique is required in order to decrease postoperative complications.
